Guidance for Prior Authorization of Waiver-Funded Supported Employment Services

The Coordination of Funding Responsibility Determination and Individual Plan outlines the Supported Employment phases that the person is expected to receive over the plan year. This can be found in the gray section on the left side of the form. The person-centered service team should address this plan and return it for any necessary changes before seeking initial prior authorization.  If changes are needed during the plan year, the person-centered team should address them and request a revision.  If no changes occur, there would be no need to seek additional team approval.  The entire Coordination of Funding process must be repeated each plan year beginning with the Coordination of Funding Information Submission Form.


Person-Centered Job Selection (PCJS)

A process of learning about a job seeker so that the information may be translated into job tasks, settings, preferences, and support needs, which will become the basis for creating a targeted job development list. Person-Centered Job Selection (PCJS) concludes with developing a Person-Centered Employment Plan (PCEP).

Supports for Community Living Waiver (SCL)

PCJS is limited to no more than 120 units of service spread over four months or less for a new PCEP; or 60 units of service spread over no more than two months for a PCEP update.  You should use procedure code T2019 with modifier U4.  Since PCJS is not an on-site supported employment service, it should not be subject to the day training/community access/personal assistance/hours of paid employment or on-site supported employment service.  Discuss the number of monthly units to be used with the employment specialist and frame the request accordingly.  Once the PCEP is completed, the person-centered service team should agree on it and upload it to the Medicaid Waiver Management Application (MWMA).  Once this is complete, end-date the PCJS prior authorization (if it has not already expired), and request the prior authorization for the next service (Job Development).

Michelle P. Waiver (MPW)

PCJS is limited to no more than 120 units of service spread over four months or less for a new PCEP; or 60 units of service spread over no more than two months for a PCEP update.  You should use procedure code H0039.  Supported Employment (PCJS) is subject to the 40 hours per week MPW service limit.  Discuss the number of monthly units with the employment specialist and reach consensus on PCJS scheduling, considering the participant’s other service needs.  You should frame the request accordingly.  It is important that you communicate the specific type of supported employment service to be provided in the comments section.  When the PCEP is completed, it should be agreed upon by the person-centered service team and uploaded to MWMA.  Once this is complete, end-date the PCJS prior authorization (if it has not already expired), and request the prior authorization for the next service (Job Development).


Job Development

The process of creating employment opportunities and securing competitive integrated employment. This includes networking with and learning about businesses to identify potential positions that match what was identified in the PCEP as closely as possible and aid in the application process. The goal is to match individuals’ skills, abilities, and interests with the employer’s work needs, either through open or negotiated positions.

Supports for Community Living Waiver (SCL)

Job Development is limited to no more than 90 units of service spread over four months or less.  You should use procedure code T2019 with modifier U5.  Since Job Development is not an on-site supported employment service, it should not be subject to day training/community access/personal assistance/hours of paid employment or on-site supported employment service.  Discuss the number of monthly units to be used with the employment specialist, and frame the request accordingly.  Job Development ends when a job is secured.  When this has been completed, the Job Development prior authorization should be end-dated, if it has not already expired, and the prior authorization for the next service (Job Acquisition with Support and Stabilization) should be requested.

Michelle P. Waiver (MPW)

Job Development is limited to no more than 90 units of service spread over four months or less.  You should use procedure code H0039.  Supported Employment (Job Development) is subject to the 40-hour per week MPW service limit.  Discuss the number of monthly units to use with the employment specialist and agree on Job Development scheduling, considering the participant’s other service needs.  You should frame the request accordingly.  It is important that you communicate the specific type of supported employment service to be provided in the comments section.  Job Development ends when a job is secured.  When this has been completed, the Job Development prior authorization should be end-dated, if it has not already expired, and the prior authorization for the next service (Job Acquisition with Support and Stabilization) should be requested.


Job Acquisition with Support and Stabilization

Direct on-the-job support to help the person reach their highest level of competence at performing job tasks and successfully integrating into the workplace culture.

Supports for Community Living Waiver (SCL)

Job Acquisition with Support and Stabilization may be utilized for up to 800 units of service and may continue until the participant becomes proficient with job tasks and has acclimated to the workplace culture at the work site.  You should use procedure code T2019 with modifier U6.  Job Acquisition with Support and Stabilization is an on-site supported employment service and is subject to the day training/personal assistance/hours of paid employment or on-site supported employment service limit of 320 units per week.  But, since the volume of Job Acquisition with Support and Stabilization cannot exceed the person’s hours of work, the relevant figure will almost always be the hours of paid employment.  It typically begins with the person receiving support 100% of the time and should decrease at a pace based upon the participant’s ability to master skills and develop working relationships with their supervisor, coworkers, and customers.  You should have ongoing discussions with the employment specialist about the number of monthly units to be used and frame the request(s) accordingly.  Job Acquisition with Support and Stabilization concludes with the development of a Long-term Employment Support Plan based upon the person’s Job Acquisition with Support and Stabilization experience that will serve as the justification for the quantity of Long-Term Employment Supports to be requested.  Once this is complete, end-date the Job Acquisition with Support and Stabilization prior authorization, if it has not already expired, and request the prior authorization for the next service (Long-Term Employment Supports).

Michelle P. Waiver (MPW)

Long-Term Employment Supports

Direct or indirect support or monitoring to help the person maintain their employment and consider opportunities for career advancement. The specific long-term supports are defined in the current Long-Term Employment Support Plan or OVR Extended Service Plan (if this is the first long-term support request following OVR funding).

Supports for Community Living Waiver (SCL)

The quantity of Long-Term Employment Supports to be prior authorized should be based upon the participant’s support needs as defined in their current Long-Term Employment Support Plan.  Update and upload the Long-Term Employment Support Plan to the person’s documents on MWMA before every Long-Term Employment Support prior authorization.  Participants transitioning from OVR funding may utilize the OVR Extended Service Plan for their initial prior authorization. It must, however, be uploaded to the person’s documents on MWMA.  You should use procedure code T2019 with no modifier. Although Long-Term Employment Supports are not always an on-site supported employment service, the hours of paid employment are subject to the day training/community access/personal assistance/hours of paid employment or on-site supported employment service limit.  Each Long-Term Employment Support Plan should include a fading plan to eventually support the person using no more than 24 units per month.  The initial prior authorization, and any prior authorization requesting more than 24 units per month, is limited to six months.  After the initial prior authorization, you may request up to the full plan year for quantities of 24 units per month or less.  

Michelle P. Waiver (MPW)

The quantity of Long-Term Employment Supports to be prior-authorized should be based upon the participant’s support needs as defined in their current Long-Term Employment Support Plan.   Update and upload the Long-Term Employment Support Plan to the person’s documents on MWMA before each Long-Term Employment Support prior authorization.  Participants transitioning from OVR funding may utilize the OVR Extended Service Plan for their initial prior authorization. However, it must be uploaded to their documents on MWMA.  You should use procedure code H0039.  Supported Employment (Long-Term Employment Supports) is subject to the 40-hour-per-week MPW service limit.  The quantity of Long-Term Employment Supports is based upon the person’s support needs as defined in their most recent Long-Term Employment Support Plan.  Each Long-Term Employment Support Plan should include a fading plan to eventually support the person using no more than 24 units per month.  It is important that you communicate the specific type of supported employment service to be provided in the comments section.  The initial prior authorization, and any prior authorization requesting more than 24 units per month, is limited to six months.  After the initial prior authorization, you may request up to the full plan year for quantities of 24 units per month or less.
