ISA Quality of Life Survey

This survey is part of a larger settlement agreement between Kentucky Protection & Advocacy and the Cabinet for Health and Family Services to move people with serious mental illness and/or substance use out of Person Care Homes (PCH) and into their own homes. Surveys are collected prior to the person moving from the PCH and six and 12 months are the person has moved. The goal is to determine if participants report a higher quality of life after moving out of the PCH.
