This happens for: <\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Supported employees under age 25: after the final Extended Services rate is paid and the OVR case is closed. <\/li>\n\n\n\n<li>Supported employees over age 25: once the Employment Stability Assessment Form is approved.<\/li>\n<\/ul>\n\n\n\n<div class=\"wp-block-stackable-spacer stk-block-spacer stk--no-padding stk-block stk-026f859\" data-block-id=\"026f859\"><style>.stk-026f859 {height:25px !important;}<\/style><\/div>\n\n\n\n<div class=\"wp-block-stackable-heading stk-block-heading stk-block-heading--v2 stk-block stk-358a49c\" id=\"what-do-i-need-to-do\" data-block-id=\"358a49c\"><style>.stk-358a49c .stk-block-heading__text{font-family:\"Inter\", Sans-serif !important;}<\/style><h2 class=\"stk-block-heading__text\">What Do I Need to Do?<\/h2><\/div>\n\n\n\n<p class=\"wp-block-paragraph\">When requesting to bill any supported employment services for Michelle P. Waiver, Supports for Community Living Waiver, or Acquired Brain Injury Waiver:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>You will need to complete the <a href=\"https:\/\/hdi.uky.edu\/setp\/wp-content\/uploads\/sites\/6\/2026\/04\/Coordination-of-Funding-Information-Submission-Form.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Coordination of Funding Information Submission Form<\/a>\n<ul class=\"wp-block-list\">\n<li>Email this form (as an editable document) to <a href=\"mailto:DDID.SupportedEmployment@ky.go\">DDID.SupportedEmployment@ky.go<\/a>v<\/li>\n\n\n\n<li>If you do not want to email an editable document, upload it to the Medicaid Waiver Management Application (MWMA) and email <a href=\"mailto:DDID.SupportedEmployment@ky.gov\">DDID.SupportedEmployment@ky.gov<\/a>. Please let them know the case number and that it has been uploaded.<\/li>\n\n\n\n<li>If requesting Long Term Support units, ensure the <a href=\"https:\/\/hdi.uky.edu\/setp\/wp-content\/uploads\/sites\/6\/2024\/10\/Extended-Services-Plan.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Extended Services Plan<\/a> (if recently completing OVR supported employment) or a Long Term Support Plan is current in MWMA.<\/li>\n\n\n\n<li>You will receive a response and be advised to contact OVR to reapply for services or to complete the <a href=\"https:\/\/hdi.uky.edu\/setp\/wp-content\/uploads\/sites\/6\/2024\/11\/Waiver-Only-Employment-Services-Prior-Authorization-Request.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Employment Services Prior Authorization<\/a> form to request payment via Medicaid waiver.<\/li>\n\n\n\n<li>If denied or an LOI is received, you may reach out to <a href=\"mailto:DDID.SupportedEmployment@ky.gov\">DDID.SupportedEmployment@ky.gov<\/a> with questions or concerns<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Note: The Authorization of Long-Term Supports is valid for the job listed on the <a href=\"https:\/\/www.chfs.ky.gov\/agencies\/dbhdid\/Pages\/scl-forms-employment.aspx\" target=\"_blank\" rel=\"noreferrer noopener\">Long-Term Support Plan<\/a> (or initial Extended Services Plan). If the individual changes jobs, complete a <a href=\"https:\/\/hdi.uky.edu\/setp\/wp-content\/uploads\/sites\/6\/2026\/04\/Coordination-of-Funding-Information-Submission-Form.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Coordination of Funding Information Submission Form<\/a> and follow the guidance given to change the Prior Authorization (PA).<\/li>\n<\/ul>\n\n\n\n<div class=\"wp-block-stackable-spacer stk-block-spacer stk--no-padding stk-block stk-b88ca4b\" data-block-id=\"b88ca4b\"><style>.stk-b88ca4b {height:26px !important;}<\/style><\/div>\n\n\n\n<div class=\"wp-block-stackable-heading stk-block-heading stk-block-heading--v2 stk-block stk-41c097e\" id=\"scl-waiver-participant-unit-allowances\" data-block-id=\"41c097e\"><style>.stk-41c097e .stk-block-heading__text{font-family:\"Inter\", Sans-serif !important;}<\/style><h3 class=\"stk-block-heading__text\">SCL Waiver Participant Unit Allowances:<\/h3><\/div>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Person Centered Job Selection: \n<ul class=\"wp-block-list\">\n<li>New Person-Centered Employment Plan \u2013 up to 120 units over not more than 4 months or the end of the person\u2019s plan year, whichever comes first. Remaining units may be requested with the person\u2019s new plan.<\/li>\n\n\n\n<li>Revised Person-Centered Employment Plan &#8211; up to 60 units over not more than 2 months or the end of the person\u2019s plan year, whichever comes first. Remaining units may be requested with the person\u2019s new plan. <\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Job Development: \n<ul class=\"wp-block-list\">\n<li>Up to 90 units over not more than 4 months or the end of the person\u2019s plan year, whichever comes first. Remaining units may be requested with the person\u2019s new plan. <\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Job Acquisition with Support &amp; Stabilization: \n<ul class=\"wp-block-list\">\n<li>Up to 800 units over the span of time necessary for the person to reach proficiency in their job or the end of the person\u2019s plan year, whichever comes first.
