To provide Supported Employment, you must become a vendor with OVR as a Community Rehabilitation Program (CRP). The application process will require the following:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li><a href=\"https:\/\/kcc.ky.gov\/Vocational-Rehabilitation\/Documents\/Directories%20and%20Publications\/Vendor%20Information\/Vendor-Application-Form(rev-3).pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Vendor Application<\/a>: This is a general application for doing business with OVR.<\/li>\n\n\n\n<li><a href=\"https:\/\/kcc.ky.gov\/Vocational-Rehabilitation\/Documents\/Directories%20and%20Publications\/Vendor%20Information\/OVR%20Vendor%20Agreement%20(rev%2004-2024).pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Vendor Agreement<\/a>: Terms that approved vendors must sign and adhere to annually to continue providing services.<\/li>\n\n\n\n<li><a href=\"https:\/\/kcc.ky.gov\/Vocational-Rehabilitation\/vendors\/VendorInfo\/Documents\/2026%20Vendor%20and%20CRP%20Manual_Final_5-29-2026%20%28rev%202%29.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Vendor Manual (CRP Manual begins on page 20)<\/a>: Provides additional information on doing business with OVR, attachments required for the CRP Vendor Application (see page 26), and additional expectations. Note the manual links you to the current manual, updated May 2026.<\/li>\n\n\n\n<li><a href=\"https:\/\/www.sos.ky.gov\/Pages\/default.aspx\" target=\"_blank\" rel=\"noreferrer noopener\">Secretary of State\u2019s Office<\/a>: Vendors must be in good standing.<\/li>\n\n\n\n<li><a href=\"https:\/\/revenue.ky.gov\/Business\/Pages\/Register-Business.aspx\" target=\"_blank\" rel=\"noreferrer noopener\">Kentucky Tax Registration<\/a>: You must apply\/register for a Sales and Use or a Consumer\u2019s Use tax account. Supported Employment Core Training Series<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>This training is offered through the Supported Employment Training Project at the University of Kentucky Human Development Institute.<\/li>\n\n\n\n<li>The training is encouraged before completing the vendor application to ensure you understand the expectations of services.<\/li>\n\n\n\n<li>The training will be required for those providing direct supported employment services prior to serving OVR consumers. <\/li>\n\n\n\n<li>Learn more and <a rel=\"noreferrer noopener\">Kentucky Small Business Development Center<\/a>.<\/li>\n\n\n\n<li>The Kentucky Small Business Development Center exists to help small businesses start, grow, and succeed. <\/li>\n\n\n\n<li>They provide no-cost, confidential business coaching, business planning, access to capital, marketing assistance, market research, and business training services.<\/li>\n\n\n\n<li>You will want to ensure that your business plan does not rely strictly on Office of Vocational Rehabilitation funding for the provision of all supported employment services. <\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading has-contrast-color has-text-color has-link-color wp-elements-a13c8ebf021ff9e96150508245b7baa8\"><strong>Choose Your Agency Name Carefully<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Carefully consider selecting an agency name that reflects employment services.<\/li>\n\n\n\n<li>If part of a larger agency, consider a separate name for your employment services division.<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading has-contrast-color has-text-color has-link-color wp-elements-4116beb74ac707d8cda37e85fa893bee\"><strong>Service Type Selection (page 2)<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>If interested in Supported Employment, check the CRP Services box only.<\/li>\n\n\n\n<li>Note: selecting multiple services increases review time (multiple departments must approve). For example, if applying for Supported Employment and Pre-ETS, Supported Employment must be approved by the CRP branch for CRP services and the Transition Branch for Pre-ETS.&nbsp;<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading has-contrast-color has-text-color has-link-color wp-elements-c6f4507e8f728ab8d1608cf1b9553c39\"><strong>Service Providers and Credentials (page 3)<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>For Supported Employment, Employment Specialists must begin the Supported Employment Core Training Series within 3 months of their start date and complete within 6 months.<\/li>\n\n\n\n<li>OVR requires that each Employment Specialist obtain 15 hours of training, including 4 hours specific to deaf, blind, or DeafBlind, annually (July-June).<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading has-contrast-color has-text-color has-link-color wp-elements-cd6efd74cab09bb0a501bee6ea88c857\"><strong>Type of Service Applying For (page 5)<\/strong><\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Common services provided by those who want to offer Supported Employment include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Traditional Supported Employment <\/li>\n\n\n\n<li>IPS Supported Employment\n<ul class=\"wp-block-list\">\n<li>Evidence-based model for individuals with serious mental illness and\/or substance use disorder<\/li>\n\n\n\n<li>Requires additional prior approval from the Division of Mental Health or the Office of Vocational Rehabilitation<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Customized Supported Employment \n<ul class=\"wp-block-list\">\n<li>Requires additional certifications of Employment Specialists (e.g., Marc Gold &amp; Associates or SETP Customized Employment Training)<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Employment &amp; Retention<\/li>\n\n\n\n<li>Pre-Vocational Services<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading has-contrast-color has-text-color has-link-color wp-elements-c166737927c8df7649907ba5eddfb391\"><strong>Populations to be Served<\/strong> <strong>(page 5)<\/strong><\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">If you are certified to provide Supports for Community Living (SCL), Michelle P. Waiver (MPW), or Individual Placement and Support (IPS), check the box for each funding source you are certified to provide. If you are not certified for these services, check all others.  <\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Important distinctions:<\/strong>\n<ul class=\"wp-block-list\">\n<li>Supports for Community Living (SCL) and Michelle P Waiver (MPW) are 1915(c) waiver programs for individuals with intellectual and developmental disabilities. To bill waiver services, agencies must be certified through the Department for Behavioral Health, Developmental and Intellectual Disabilities.<\/li>\n\n\n\n<li>IPS is an evidence-based practice of supported employment for individuals with serious mental illness and\/or substance use disorders. Agencies must be certified through the Division of Mental Health or Office of Vocational Rehabilitation.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading has-contrast-color has-text-color has-link-color wp-elements-633fc8de98c947687522fd18a09e5671\"><strong>Hours of Operation<\/strong> <strong>(page 5)<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>You may list your standard office hours such as 8:00 AM &#8211; 4:00 PM<\/li>\n\n\n\n<li>Supported Employment services will need to be flexible and based on individual needs, including evenings and weekends<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading has-contrast-color has-text-color has-link-color wp-elements-8eb660ee87d247f83716181d0bdf4ba3\"><strong>Admissions<\/strong> <strong>(page 6)<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Clearly define how referrals will be reviewed and accepted (be sure to review information on page 28 of the Vendor Manual).<\/li>\n\n\n\n<li>Consider:\n<ul class=\"wp-block-list\">\n<li>Who reviews referrals?<\/li>\n\n\n\n<li>What criteria determine acceptance or denial?<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Example<\/strong>:\n<ul class=\"wp-block-list\">\n<li>The Supported Employment Supervisor reviews referrals from OVR to determine service fit. If appropriate, the case is assigned to an Employment Specialist, who will contact the job seeker within 5 days to schedule an appointment. <\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading has-contrast-color has-text-color has-link-color wp-elements-8b8bae0c0fafd309019072a9af09e787\"><strong>Describe your organization\u2019s mission and why you desire to provide SE services<\/strong> <strong>(page 6)<\/strong><\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">It is important to include how you will ensure supported employment services lead to <a href=\"https:\/\/www.dol.gov\/agencies\/odep\/program-areas\/cie\" target=\"_blank\" rel=\"noreferrer noopener\">Competitive Integrated Employment (CIE)<\/a>. CIE is defined in the Vendor Manual.<\/p>\n\n\n\n<h4 class=\"wp-block-heading has-contrast-color has-text-color has-link-color wp-elements-b961615d414799e163ef916487524d11\"><strong>Describe how you plan to Address\/Assure Integration at the Job Site<\/strong> <strong>(page 7)<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>They want to ensure you understand the importance of how integration on the job site is essential and that you have the forethought to ensure it occurs.<\/li>\n\n\n\n<li><strong>Example<\/strong>:\n<ul class=\"wp-block-list\">\n<li>Our agency will ensure integration at the job site by promoting meaningful inclusion of the individual within the workplace culture and environment. We will work collaboratively with both the individual and the employer to foster natural supports and relationships that contribute to successful, long-term employment.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading has-contrast-color has-text-color has-link-color wp-elements-66e25b5f610783bea9e9e4f68e4ca35f\"><strong>Population You Plan to Serve (page 7)<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Restricting populations (e.g., only accepting individuals with specific funding or diagnoses) may reduce referrals<\/li>\n\n\n\n<li>Describe if your agency intends to limit who you will serve and why. Otherwise, you can indicate that you are open to serving all people, regardless of impact of disability.<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading has-contrast-color has-text-color has-link-color wp-elements-e07351ca248f6b214115c4ad8986bdbb\"><strong>Consumer Satisfaction<\/strong> <strong>(page 7)<\/strong><\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Consider annual satisfaction surveys of individuals served, funding sources, employers, etc. <\/p>\n\n\n\n<h4 class=\"wp-block-heading has-contrast-color has-text-color has-link-color wp-elements-639b513773845fb0b9f1903d11d1672e\"><strong>Funding for Ongoing Services<\/strong> <strong>(page 8)<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>OVR funding is short-term and will end upon case closure, likely 90 days after reaching stable employment.<\/li>\n\n\n\n<li>Supported Employment is long-term, and the CRP is expected to provide extended services throughout the term of employment. The minimum expectation is two contacts per month face-to-face at the job site.<\/li>\n\n\n\n<li>CRPs are expected to be able to fund and provide extended services knowing they will need a funding
