HDI Seminar Series: Countdown to a Successful Launch: An Exploration of Secondary Transition Data, Planning and Outcomes

Learning Objectives: This seminar will be comprised of a panel of experts in various areas of secondary transition for youth with disabilities. The panel will be moderated by the director of the Kentucky Post School Outcomes Center who will provide data and context for panelists to discuss. Panelists will describe best practices for improving life outcomes for people with disabilities and provide information on resources for parents, professionals, and persons with disabilities. Learning Objectives 1. Participants will learn how to help youth effectively plan for life after high school in terms of employment, continuing education and independent living. 2. Participants will understand current data regarding post-school outcomes for youth with disabilities. 3. Participants will see the results of best practices related to transition.
